We're excited to welcome your child aboard the Pink Pamper Van! For a safe and sparkle-filled party, please review and consent to the following terms. Please complete all fields below to ensure we have the correct information for your child.

- Warm water foot spa soaks
- Relaxing Shiatsu foot massagers
- Gentle, kid-friendly face masks

- Treats and sugar free refreshments
Activities & General Permission
By signing this form, you confirm that:
•You are the parent or legal guardian of the child named on this form.
• You are happy for them to take part in pamper activities in the Pink Pamper Van (such as mini manicures, pedicures, gentle facials and light massage).
• You understand that all activities are for fun and relaxation only and are not medical or beauty treatments.
• You will tell us about any health conditions, allergies or additional needs before the party so we can keep your child safe.

The information on this form is for booking administration and health and safety only. We never sell your personal details and only keep them as long as legally required for insurance and safeguarding.

Health, Safety & Responsibility
We provide a safe, professionally managed environment for every mini guest on the Pink Pamper Van. To keep the magic alive and everyone safe, please note the following:
- Activities and products carry minimal risks of skin sensitivity or minor injury.
- Your child participates in the pamper experience voluntarily.
- We hold standard UK liability insurance for your peace of mind.
- Pink Pamper Van is not responsible for unforeseen reactions not caused by negligence.

Capture the Sparkle: Photo & Video
We love sharing party smiles! To help us celebrate every magical moment, we capture photos and short clips for our social media and website. Under UK GDPR, unless expressed otherwise you give consent for your child’s image being used in:

- Our website content and galleries
- Printed promotional materials (leaflets and brochures)
- Business marketing and advertisements
